One Door Mental Health- CPS Connector Hub SWS welcomes you to their 2022 program information and launch day. About this event Come unpack what Connector Hub offers and understand how the program can support people with severe mental health challenges. We welcome all local community services and community members. There is potential for local services to network and to explore other mental health services in South Western Sydney. Connector Hub is a commonwealth psychosocial support program funded by the Primary Health Network South Western Sydney and is hosted by One Door Mental Health(Campbelltown, Camden, Wingecarribee and Wollondilly).

The program offers:
-One-to one peer and recovery support -One-to -one service navigation and service linkage -Psychosocial groups -for social inclusion, getting out into the community and connecting -Support to apply or re-apply for the NDIS Anyone can refer an eligible person to the program (with their permission) including self-referrals, carers, family or friends and health professionals.
