SWS You in Mind Peer Support Program

 

The South West Sydney (SWS) Peer Support Program (PSP) is a SWS Primary Health Network (PHN) funded initiative. A team of Peer Workers will support People to access the You In Mind program, and work in close collaboration with You In Mind Clinicians throughout the South West Sydney area to:

• Support the recovery of people experiencing low, moderate and high mental health through a non-clinical approach;
• Create a space of hope, optimism and inclusion for those experiencing a mental illness, as well as, their carers and families;
• Support person-centred access and engagement with primary mental health services in line with integrated stepped-care model;
• Contribute to an increased embedding of person-first, recovery-orientated and family/carer inclusive approaches;
• Promote the voice of people with lived experience of mental illness. Thus,
• Increasing the Peer Worker and lived experience workforce engaging with primary mental health services in SWS.

Only people accessing You In Mind practitioners can be referred to the Peer Support Program.

Eligibility

Only Individuals accessing Clinicians of the South West Sydney You In Mind program will be eligible to access PSP.

PSP will target:
• You in Mind participants at risk of disengaging with treatment and would benefit from additional support to continue with Clinician;
• All You In Mind participants who step up to a higher level of clinical support;
• Participants of You In Mind with psychosocial barriers and needs that a Peer Worker could provide additional support and service connection. This could include: cultural background, trauma, insecure housing, financial concerns, social isolation, family circumstances and domestic violence.
• Participants of You In Mind who are willing to participate in PSP.

Referral and Allocation

Peer Work may be suggested when identified that an individual has goals that require support outside of a clinical session.

Referrals to PSP can ONLY be initiated by:
• You In Mind Clinicians


GP’s, SWSPHN Mental Health Intake, Peer Worker and other service providers who feel Peer Support would be of benefit to an individual on initial referral to You In Mind, please suggest in the referral to PHN.
